If MUMC is choosing Angels for you, Please list any preferences you have for your Angels (ex. ages, gender, etc.). Also indicate how many Angels you would like to adopt.

Description

Please fill out all sections of the form. Angels with the same number are from the same family. The letters identify individual children within the family. If you want to adopt an entire family, be sure to grab each Angel of that number.

All gifts must be unwrapped. Group the gifts together and label with the Angel ID number. Return the gifts to 3851 McKinney St by December 15. For any questions, email heather@melissaumc.com.
